What is the Community Care Team?
The Community Care Team (CCT) is a group of healthcare professionals who provide expertise, guidance and support in managing your chronic disease or health condition. These professionals work with you to set goals and develop strategies to manage and improve your overall health and wellbeing.
Your CCT may include:
- Nurse Care Manager
- Community Resource Social Worker
- Certified Diabetic Nurse Educator
- Community Health Worker
How can the CCT help your health?
- Nutrition, exercise and health coaching
- Diabetic education
- Substance abuse screening or referrals
- Medication management and reconciliation
- Behavioral/mental health support
- Dental care coordination
What support services does the CCT provide?
After a social needs assessment has been conducted, you will be connected to the following community and financial services (if needed):
- Transportation
- Financial Support
- Employment
- Housing
